Incident reporting systems are ubiquitous, despite a lack of evidence supporting their utility in identifying errors and quality of care problems. This study conducted in Canadian hospitals compared the frequency of errors identified through incident reports with that of errors identified using trigger methodology. As shown in prior research, incident reports identified only a small proportion of adverse events. A framework for analyzing and responding to incident reports was presented in an earlier study.
