Drug errors and related interventions reported by United States clinical pharmacists: The American College of Clinical Pharmacy Practice-Based Research Network medication error detection, amelioration and prevention study.

This analysis of voluntary error reports submitted by clinical pharmacists found that nearly half of the errors were intercepted by pharmacists before the error reached the patient and that clinicians frequently accepted pharmacists' recommendations for treatment changes.
