This survey study of junior surgical residents in the United States and Canada assessed perceptions of patient handoffs. Errors related to handoffs were common: more than 60% of respondents from each country said they had received an incomplete handoff that led to patient harm. Standardization of verbal handoffs was frequently cited as the most effective way to improve handoff safety.
