A culture of safety is a cornerstone of safe patient care. This review summarizes findings from 18 articles on the topic of safety culture in radiation, medical, and general oncology. Heterogeneous study designs potential for bias prevented definitive recommendations, but incident learning systems appear to be a good first step for oncology centers to improve safety culture.
