As part of their ACGME requirements, medical residents are expected to participate in patient safety actives including root cause analysis (RCA) and the disclosure of patient safety events. This paper describes the implementation of a brief RCA-focused simulation activity for residents at one academic medical center and reports improvements in resident confidence and knowledge.
