This persistent problem calls for new approaches to improve medication delivery, ordering, and administration for patients who take 10 or more medications. This commentary suggests engaging pharmacists in the transitions of care for these high-alert patients and details one organization's strategy to reduce readmissions related to medication problems in this patient population.
