This pre–post study of a standardized handoff intervention in an internal medicine resident continuity clinic resulted in fewer patients lost to follow-up and decreased acute care utilization around the time of provider handoff. The authors conclude that identifying and prioritizing follow-up for high-risk patients improves year-end handoffs for graduating residents.
