This collection of surveys seeks to assess the attitudes toward safety in a variety of clinical settings. This project was supported by Agency for Healthcare Research and Quality (AHRQ) Grant # 1PO1HS1154401. The use of this survey instrument is discussed by Pronovost in a 2003 article.
