Crossing to safety: transforming healthcare organizations for patient safety.

The authors argue that health care organizations have fallen short of safety goals. To support progress, they suggest a stronger focus on building a blame-free culture of patient safety, providing leadership that supports safety work through infusing it into daily practice, and implementing communication infrastructures that support safe care.
