When data go AWOL: filling the clinical gaps.

This article responds to a 2005 study, which discovered missing clinical information in more than 13% of patient visits to 32 primary care practices. Physicians interviewed suggest electronic medical records as a possible solution to this problem, as well as other, low-tech options.
