Factors associated with reported preventable adverse drug events: a retrospective, case-control study.
Beckett RD, Sheehan AH, Reddan JG. Ann Pharmacother. 2012;46:634-641.

This study analyzed more than 4300 medication errors and noted that 4% were preventable adverse drug events. Institute for Safe Medication Practices high-alert medications and 30-day readmission were associated with increased risk of harm.
