Patient safety in intensive care: results from the multinational Sentinel Events Evaluation (SEE) study.

The authors conducted an analysis of adverse events in intensive care units (ICUs) worldwide and found that problems with medications, indwelling lines, airway, and equipment failures occur frequently in ICUs.
