Diagramming patients' views of root causes of adverse drug events in ambulatory care: an online tool for planning education and research.

The investigators used a root cause analysis approach to interview primary care patients, who identified 164 causes for adverse drug events (ADEs). The authors diagrammed the relationships of causal factors to inform education and practice changes that address ADEs in ambulatory care.
