Targeted chart review of pediatric patient safety events identified by the Agency for Healthcare Research and Quality's Patient Safety Indicators methodology.

The Agency for Healthcare Research and Quality's Patient Safety Indicators (PSIs) are screening tools that can be applied to administrative data to identify potential patient safety problems. Although they have been validated in analyzing data from a variety of settings, this study sought to determine their utility in analyzing pediatric patient safety. While the PSIs generally performed well compared to chart review, certain specific PSIs (such as failure to rescue) did not accurately identify patient safety problems in hospitalized children. This confirms the findings of a prior study in this area.
