Adverse Health Events in Minnesota: Ninth Annual Public Report.
St. Paul, MN: Minnesota Department of Health; January 2013.

The National Quality Forum has defined 27 "never events"—patient safety problems that should never occur, such as wrong-site surgery and patient falls. Since 2003, Minnesota hospitals have been required to report instances of such events. In the 2012 report, 314 such events were reported, and though this number is essentially unchanged from the previous year, there were more reported incidents of harm. The reports from previous years are also available.
