Using an interactive voice response system to improve patient safety following hospital discharge.

Patients in this study received an automated phone call 48 hours after hospital discharge to assess for new symptoms, medication problems, or need for clinical assistance. One quarter of patients reached by the system indicated new symptoms, half of which required intervention from a nurse.
