Hospital readmissions have emerged as a national policy priority, in part spurred by financial penalties for high readmission rates under the Affordable Care Act. This study used the care transitions survey to assess the relationship between parents' perceptions of their child's hospital discharge and readmission. Unplanned 30-day readmissions were significantly more likely when parents did not believe that their child was "healthy enough to leave the hospital." Of note, more than 95% of parents in this survey felt they had all the information needed to take care of their child upon discharge. The National Quality Forum has endorsed a set of strategies to prevent adverse events after discharge and Project RED researches strategies for reducing readmissions.
