Barriers to self-reporting patient safety incidents by paramedics: a mixed methods study.

This survey study identified fear of professional consequences as the major barrier for paramedics in voluntarily reporting a patient safety event in the prehospital setting. The authors suggest that improving safety culture is needed in order to increase event reporting.
