Double checking: a second look.

Manual double checking of high-risk medication administration is a standard safety practice at most hospitals, despite limited data regarding its effectiveness. This qualitative study found that health care providers had differing conceptions of what double checking actually meant in practice, and they perceived that efficiency was impaired significantly by this process.
