Implementation of an electronic medication reconciliation tool led to fewer medication discrepancies, as measured by voluntary reporting. Although this study may have over-estimated the effectiveness of the tool due to under-reporting of discrepancies, it demonstrates a potential way to enhance medication reconciliation, a key patient safety practice that remains incompletely implemented.
