Culture, hierarchy, and lack of opportunity can hinder reporting of concerns and mistakes. This commentary advocates for reporting of concerns to be considered a moral requirement, incorporating education and awareness regarding challenges to reporting concerns, and promoting a culture that is open to feedback.
