Despite the widespread use of incident reporting and other error detection systems, there remains little consensus about how to best use the information gathered from reporting systems to improve safety. This study combined review of literature from health care and other high-risk industries with structured interviews to develop a framework for high-yield feedback mechanisms to improve quality and safety.
