Failure to order an appropriate diagnostic test and inadequate follow-up planning can contribute to diagnostic failures in primary care. This commentary recommends actions general internists should take to improve diagnosis in primary care, such as leading efforts to reduce diagnostic error, enhancing reasoning and examination skills, and working to augment measurement around diagnosis.
