Error reporting plays an important role in preventing future errors and harm, but research has shown errors are frequently underreported. This systematic review mapped facilitators and barriers to reporting medication errors to the Theoretical Domains Framework (TDF). Beliefs about consequences emerged as both a facilitator and barrier to reporting. Emotions, environmental context and resources, and knowledge emerged as additional barriers.
