Implementation of a patient safety program at a tertiary health system: a longitudinal analysis of interventions and serious safety events.

This study describes the implementation of a bundle of high-reliability interventions in a health system. With leadership engagement, the health system initiated team training, incident reporting, and root cause analysis. The rate of serious safety events declined following implementation of these evidence-based practices.
