Incident reporting behaviours following the Francis report: a cross-sectional survey.

Archer G, Colhoun A. Journal of Evaluation in Clinical Practice. 2017;24.

Participants cited time, the reporting process, culture, organizational issues, and inadequate feedback as barriers to reporting.
