Morbidity and mortality: delays in my patient’s cancer care.

System failures contribute to access issues that result in patient harm. This commentary shares insights gleaned through a physician’s root cause analysis (RCA) of a cancer care delay incident that diminished a patient’s quality of life. Medicaid enrollment, referral requirements, and poor care coordination all degraded the safety of care.
