Patient safety incidents associated with equipment in critical care: a review of reports to the UK National Patient Safety Agency.

The National Patient Safety Agency in the United Kingdom maintains a voluntary incident reporting system. Approximately 1 in 12 reported incidents in critical care units was related to equipment problems, including more than 500 incidents of overt equipment failure during a 6-month period.
