This quality improvement intervention engaged physician leaders and frontline clinicians in leading chart audits. Participants utilized a structured tool to determine whether specific safety procedures, such as medication reconciliation, were performed. This process was successfully implemented and supported improvement efforts in areas that were resistant to previous approaches to change, suggesting that simultaneously engaging leadership and frontline clinicians may be effective in enhancing safety culture.
