The authors relate personal experiences with physician error, including the professional and emotional consequences. The authors argue for increased discussion of errors among physicians, both as an educational exercise and as a means of increasing collegial support. The authors suggest physician leaders in an organization can improve organizational culture by setting an example of open disclosure and describe resident discussion groups they have convened to promote this behavior as a part of training.
