Transitions in hospitals are vulnerable to medication errors due to poor communication. Spotlighting medication reconciliation as an improvement strategy, this review recommends that pharmacists utilize a standard process to ensure that the correct medication information follows patients throughout their hospital stay and discharge to prevent errors.
