High-reliability organizations learn from failure and identify systems changes to improve patient safety. This systematic review identified several available methods to prioritize recommendations following analyses of adverse events (AEs), but none of these methods met the three essential features essential to learning from AEs and improving care (user-friendly, validated, and based on objective features).
