Reducing medication errors in critical care patients: pharmacist key resources and relationship with medicines optimisation.

In this study, pharmacists achieved consensus about how to most effectively reduce medication errors in critical care. They ranked interventions by importance and assessed differences between high-, medium-, and low-resource units. Pharmacists in high-resource units optimized medications more often, though error rates were similar across critical care units studied. A WebM&M commentary reviewed the power of pharmacists to improve medication safety.
