Wrong-patient errors have the potential to cause serious patient harm. This study analyzed retract-and-reorder (RaR) medication orders to examine risk factors for wrong-patient orders in the emergency department. Findings indicate that RaR events are more likely when patients share the same sex and have exam rooms located near each other. Other factors, such as patient name, birthdate, and age, were not significantly associated with RaR events.
