Relating an account of medical student whose failure to speak up about uncertainty jeopardized a patient's safety, this commentary illustrates how the hidden curriculum, disruptive behaviors, and hierarchy influence willingness to speak up about safety hazards. The authors propose strategies for organizations to address this issue, including eliciting feedback from students and promoting communication strategies.
