This quality improvement initiative found suboptimal adherence to surgical safety checklists. The authors suggest that wide variation in compliance may explain conflicting results between randomized trials of checklists and subsequent observational post-implementation studies, and this finding points to the need to study implementation, not just effectiveness, of patient safety strategies.
