Blame culture in health care settings hinders incident reporting and safety improvement. This article describes an initiative built on the CUSP model to engage nursing staff in transparent discussion of safety concerns. The initiative applied high reliability principles of developing safety awareness and learning from problems and employed huddles and leadership safety rounds to illustrate the commitment to a supportive reporting environment.
