Inappropriate exposure to radiotherapy can result in significant patient harm. This commentary explores safety incidents in radiotherapy and highlights underdosing and positioning problems as important considerations for radiology practice. The authors review existing guidance and evolving United Kingdom policy efforts to enhance the safety in radiotherapy.
