This survey study of pediatric anesthesiology fellows found a high rate of self-reported medication errors overall. The rate of reported errors did not differ among respondents with varying levels of supervision. The authors highlight the need to investigate and reduce the high rates of reported perioperative medication errors.

Care transitions increase risk for medication errors. This meta-analysis found that pharmacist support during care transitions reduced medication errors and postdischarge emergency department visits. These results support a role for pharmacists in medication safety interventions for patients undergoing care transitions.

Several commonly used anesthesia medications are among the 140 medications currently considered to be in short supply. Data from MEDMARX reveals that shortages have been implicated in many cases of prescribing errors, often when one concentration of medication was substituted for the unavailable formulation.
