Resources are identified using the National Library of Medicine’s Medline database, various news and content aggregators, and the expertise of the AHRQ PSNet editorial and technical teams.

Leapfrog Group.
This website offers resources related to the Leapfrog Hospital Survey investigating hospitals' progress in implementing specific patient safety practices. Updates to the survey include increased time allotted to complete computerized provider order entry evaluation, staffing of critical care physicians on intensive care units, and use of tools to measure safety culture. The AHRQ Surveys on Patient Safety Culture™ (SOPS®) Hospital Survey and accompanying toolkit were developed to collect opinions of hospital staff on the safety culture at their organizations. Participating hospitals will be able to measure patient safety culture in their institutions and compare results with other sites. Data collection for the latest submission period is closed.

The Centers for Medicare and Medicaid Services (CMS) provides consumers with publicly available information on the quality of Medicare-certified hospital care through this Web site. The site includes specific information for both patients and hospitals on how to use the data to guide decision-making and improvement initiatives. Most recently, listings from the Hospital-Acquired Condition Reduction Program (HACRP) and data on Department of Veterans Affairs hospitals were added to the reports available.

The American Society for Dermatologic Surgery Association and the Northwestern University Department of Dermatology.

Voluntary reporting systems collect adverse event data to inform improvement and education efforts. This site provides a platform for physicians and their staff to submit adverse experiences associated with dermatologic surgery equipment, medications or biologics.

Centers for Medicare and Medicaid Services.

The Centers for Medicare & Medicaid Services (CMS) support transparency through the provision of publicly available information on the quality of health care service in the United States. This portal enables access to comparative quality and safety data from doctors & clinicians, hospital, nursing home, home health, hospice, inpatient rehabilitation facilities, long-term care hospitals, and dialysis facilities to support informed consumer health care provider selection activities.

The Patient Safety Organization (PSO) program seeks to gather and analyze nonidentifiable patient safety incident data to track concerns and reduce risks. This website provides data submitted from PSOs and other organizations in reports, chartbooks, and dashboards. This website was built by the team that developed and maintained the AHRQ National Guideline Clearinghouse, which is no longer available.

This commercial database tracked both adverse drug reactions and medication errors submitted by participating hospitals. Subscribers had access to national data, which can be used for trend analysis and internal benchmarking. The system was transitioned to Quantros in 2008.

This searchable listing of the American Society of Health-System Pharmacists policy and guideline collection provides user access to various content areas relevant to safe inpatient medication administration. The 2018 enhancements include a revision of the "ASHP Guidelines on Preventing Medication Errors in Hospitals".

This interactive tool identifies, tracks, analyzes, and compares statistics on hospital care. It is part of the Healthcare Cost and Utilization Project (HCUP). With HCUPnet, users gain easy access to all national and selected state statistics about hospital admissions. These statistics can provide insight into quality of care issues.

This website provides consumers with detailed information about the past performance of every Medicare- and Medicaid-certified nursing home in the country. Additional resources available at the site help with other nursing home selection issues. Information is provided in English and Spanish. In 2020, the website was renamed Care Compare, with links to compare hospitals, hospice, and other care providers.

These databases enable research on a broad range of health policy issues, including cost and quality of health services, medical practice patterns, access to health care programs, and outcomes of treatments at the national, state, and local market levels.

This web-accessible database provided access to evidence-based quality measures and measure sets. The mission of the National Quality Measures Clearinghouse (NQMC) was to provide practitioners, health care providers, health plans, integrated delivery systems, purchasers, and others an accessible mechanism for obtaining detailed information on quality measures and to further their dissemination, implementation, and use in order to inform health care decisions. Funding for the program ended in 2018.
American Society of Health-system Pharmacists; ASHP features complete, easy-to-read information on more than 800 drugs. The site helps patients find the important information they need to use medications safely and effectively.
