These resources come in a variety of formats, including literature, research, tools, and Web sites. Resources are identified using the National Library of Medicine’s Medline database, various news and content aggregators, and the expertise of the AHRQ PSNet editorial and technical teams.

This article describes an effort to help hospitals enhance discharge planning, medication management, patient and family engagement, care transition, and communication as elements of a state-wide collaborative to reduce readmissions. The program received a 2013 Eisenberg Award.
Dr. Eric Coleman has enhanced care transitions by developing tools and processes to improve communication and reduce readmissions. The MacArthur Foundation has selected him as a 2012 Fellow and recipient of $500,000 (often called a "genius grant").
