This study compared two strategies for managing patients on warfarin therapy using a retrospective cohort analysis. During a six-month period in a community setting, patients receiving pharmacist-driven care coordination compared to usual care experienced fewer adverse events requiring hospitalization. The authors suggest that similar programs may be effective at reducing complications from warfarin therapy.
