In this study, clinical pharmacists reviewed an electronic dashboard displaying potentially inappropriate medications in hospitalized elderly patients. The dashboard flagged approximately one-fourth of patients as being at increased risk for medication–related adverse events. Pharmacists made direct point-of-care recommendations, which clinicians acted on 78% of the time.
