Pharmacist medication review can uncover drug-related problems such as adverse reactions and lack of medication understanding or adherence. In this study, pharmacist drug review of older adults found frequent concerns about medications. According to a 3-month follow-up survey, the majority of patients had implemented the pharmacist's recommendations. This work argues for enhancing pharmacist medication counseling to older adults.
