After introducing a web-based voluntary incident reporting system in an academic emergency department, physician-reported incidents rose from 81 events in 2009 to 561 events in 2012. A prior PSNet perspective discussed incident reporting systems and the importance of not only increasing reporting rates but also following through with safety improvements.
