Human factors can contribute to failure in any activity. This review explores reasons clinicians remain reluctant to disclose error, such as shame, insufficient disclosure skills, and inadequate coping mechanisms. The authors suggest that systems and relationships can be developed to help address these barriers and learn from outcomes associated with mistakes.
