The patient safety movement continues to achieve successes, despite persistent and new challenges. This review explores the trajectory of the work and emphasizes the importance of a systems approach for improvement efforts. The authors highlight crew resource management as a key strategy to develop skills required to sustain safety both across the health care system and in surgical specialties.
