ASH Clinical Practice Guidelines on Venous Thromboembolism.
Approach to Improving Safety
Setting of Care

Key recommendations include low-molecular-weight heparin as the preferred agent when medication prophylaxis is indicated and screening of all hospitalized patients for venous thromboembolism risk and bleeding.
