Sources and types of discrepancies between electronic medical records and actual outpatient medication use.

This study highlights the discrepancies that may occur when conducting medication reconciliation in the outpatient setting and within an electronic health record (EHR). The majority of discrepancies noted by investigators were attributed to medications still listed in the EHR that the patient was no longer taking.
