Patients have three roles in improving patient safety: helping to ensure their own safety, working with health care organizations to improve safety at the organization and unit level, and advocating as citizens for public reporting and accountability of hospital and health system performance. The following case illustrates how patients can help ensure their own safety.

American Society of Health-System Pharmacists.
The Award for Excellence in Medication-Use Safety is a national competition that honors outstanding leadership, teamwork, innovation, and measurable results in patient safety improvements. The award is designed to recognize a pharmacy-led, multidisciplinary team effort. The winner of the award receives $50,000, and two finalists receive $10,000 each. The process for submitting proposals for the 2017 funding cycle is closed.

Michael Cohen, President of the Institute for Safe Medication Practices, has focused his career on preventing medical errors with innovative and elegant solutions. The MacArthur Foundation has selected him as a 2005 Fellow and recipient of a $500,000 "genius grant."
